YOU WILL ALSO NEED TO BRING IN A COPY OF YOUR BIRTH CERTIFICATE
|After you have completed all these steps, bring the completed packet to the Superintendent's office. Your Work Permit will be prepared for the Superintendent's signature. Once we have completed it, we will notify you. You may then come in and sign it. A copy will be made for our files and the original will be given to you to give to your employer.
