New Student Registration

Information for NEW student enrollments.

There is a two step process to enroll your student into Bradford Schools.

STEP 1: ONLINE REGISTRATION THROUGH FINAL FORMS

When you click on the Final Forms button below, it will take you to a page that will allow you to create an account. This account will allow you to:

  • Register your student with Bradford
  • Update your information throughout the school if any changes occur.

Once you create an account, it will take you to the online forms. Final Forms will save your information and  you can return to it at later date so you do not have to complete the registration process in one sitting.

If you do not have internet access, the Bradford Public Library provides computer access. The library is located at 130 East Main Street in Bradford and the phone number is (937) 448-2612. If you are not able to utilize the library, please contact the school and we will assist you.

The Online Registration process has to be completed before your student can be enrolled in the school.

STEP 2: SCHEDULE AN ENROLLMENT APPOINTMENT

When you are finished completing the online enrollment through final forms, please contact the school to schedule an enrollment appointment.

For students in K-5 Grade: Please contact Mrs. Tab Canan, the elementary secretary at (937)448-2811 or email her at [email protected] to schedule an appointment.

For students in 6-12 Grade: Please contact Mrs. Amy Gade, the junior high/ high school secretary at (937)448-2719 or email her at [email protected] to schedule an appointment.

The appointment is used to collect the required documents and will be held in the Front Office. Please use the entrance off of State Route 721 in the front of the building. This meeting should last no more than 15 minutes. Parents should bring the following information when they attend:

  • Certified Birth Certificate
  • Driver License or other form of identification
  • Any special education/504 documents
  • Immunization records
  • Proof of residence (lease, deed or utility bill)
  • Custody paperwork if applicable
